Standards for voluntary termination of a license are as follows:

1. Providers that voluntarily close or surrender their Family Foster Care for Adults License should notify the County Social Service Agency or the agency that completed the licensing study, residents, and the resident's representative, if any, in writing at least thirty days prior to the termination date.
2. The agency that completed the licensing study shall immediately notify the Human Service Center and the Program Administrator at the Division of the providers’ intent to voluntarily terminate their license.
